Soluna Care Network – Terms of Service

By enrolling in services with Soluna Care Network (“Soluna,” “we,” “our”), you (“Client,” “Member”) agree to the following Terms of Service.

1. Services Provided
Soluna Care Network provides care navigation, coordination, and support services for aging adults and their families. Services may include:
• Care guidance and planning
• Coordination of services and resources
• Placement assistance (no cost to client)
• Ongoing membership-based support
• Virtual nurse support and, where available, in-home nurse visits
Soluna is not a medical provider, home health agency, or hospice provider, and does not provide direct medical treatment or emergency services.

2. Membership & Fees
Care Onboarding & Setup Fee:
A one-time $99 fee is charged for onboarding, intake, and initial care coordination setup.
Monthly Membership:
Membership is billed at the agreed monthly rate (e.g., $279 or $329 depending on enrollment period).
• Membership begins on the agreed start date
• Fees are billed automatically via Stripe
• Membership continues until canceled

3. Founding Member Terms (if applicable)
Clients enrolled during the founding period may receive locked-in pricing as communicated at time of enrollment. This rate remains in effect while membership remains active.

4. Payment & Billing
By enrolling, you authorize Soluna to:
• Charge your payment method on file for all applicable fees
• Automatically bill monthly membership fees
• Retry failed payments
Failure to maintain a valid payment method may result in suspension of services.

5. Cancellation Policy
• Membership may be canceled at any time
• Cancellation must be submitted prior to the next billing cycle
• No prorated refunds for partial months
Services remain active through the end of the current billing period.

6. Refund Policy
• The $99 onboarding fee is non-refundable once services have begun
• Monthly membership fees are non-refundable once billed
Exceptions may be made at Soluna’s sole discretion.

7. Client Responsibilities
Clients agree to:
• Provide accurate and complete information
• Participate in recommended care planning
• Make final decisions regarding care and services
Soluna provides guidance and recommendations, but all decisions remain the responsibility of the client and/or family.

8. No Medical Advice / Emergency Services
Soluna does not provide medical diagnosis or treatment.
• Any clinical input provided is for guidance only
• Always consult your physician for medical decisions
• In case of emergency, call 911

9. Communication & Consent
By enrolling, you consent to:
• Communication via phone, text, and email
• Use of secure platforms for communication
While Soluna utilizes HIPAA-compliant systems, standard messaging may carry inherent risks.

10. Limitation of Liability
To the fullest extent permitted by law:
Soluna Care Network shall not be liable for:
• Decisions made by clients or families
• Outcomes related to third-party providers
• Indirect or consequential damages
Services are provided as guidance and coordination only.

11. Third-Party Services
Soluna may refer or coordinate with third-party providers.
• Soluna does not own or operate these providers
• Clients contract directly with third parties
• Soluna is not responsible for their services
​
12. Modifications
Soluna reserves the right to update these Terms of Service at any time. Continued use of services constitutes acceptance of any updates.
